Optical Plan

You must contact the Welfare Fund Office to obtain an optical voucher.  Call Savasta and Co. at (212) 308-4200.

You are entitled to optical services once every two years.  The benefit is $100.

You will be sent lists of the various optical networks which the Welfare Fund uses.  You can use any participating store for your optical service.

If you do not use a participating provider, the optical reimbursement will be $100 per patient.
